Puzzle Description:
Hubble Space Telescope, launched in 1990. Its primary mirror was inadvertently made 2.2 thousandths of a millimeter too flat, causing severely flawed images. A corrective lens was installed during the first servicing mission in 1993, restoring the telescope to perfect vision.
