Pour Amaretto and Bacardi into a glass set it on fire and drop it into a beer. Watch out for the heat. It is called Flaming Doctor Pepper.

I see the flames, but don't understand the rest

This is how I see it:

Top-left/green is a jalapeno or serrano pepper (or the like)... on fire...

Bottom&right/blue&black is a stethoscope... also on fire of course... leading to what is presumably a heart (bottom-left/red)... very Doctor-esque imagery.

Got the flames, got the doctor references, got the pepper... All comes together. :)
