Actually, it's absolutely nothing. It's just a silly comparison to how they depict dead spiders and bugs with their feet in the air.

If I know kids' jokes, the image in #2 is a brain eater.
The question is "What's this?"
Answer: "A brain eater"
Then you put it on the person's head and ask
"What's this?"
Answer: "A brain eater starving"

Kids today are also completely unaware of "what's that on your shirt?" (you say the phrase while touching the center of their chest with your pointer finger, like you're pointing at a stain, then when the kid looks down, you whip your finger up quickly to boop them on the nose)
