On the figure standing there should be a longer handle on the object being held. When I redid it I somehow missed making it longer.

Or how about when you're solving a rather large puzzle and you are filling in the last square (so you think) and waiting for the voila! moment only it doesn't happen? Somewhere you have missed a square and have managed to darken all the numbers on the edges so that doesn't help you and your eyes are tired and you look and look for that empty square. Or maybe I'm the only klutz this happens to.

I am no longer surprised when someone has a different take on a puzzle than the one I intended. That's one reason I enjoyed the webpbn contest so much. I love seeing all the different interpretations of the same subject.
