Well, the part of the photo that looks like the puzzle is the "roundel"...which is a disc-shaped method of ID'ing the country of origin for military aircraft...AND it is 7 letters long.#15: Joe (infrapinklizzard) on Sep 13, 2020 [SPOILER]
Oh! Joe said anagram! knotted is a reference to it being an anagram? "under" + O.L. = "roundel"...and "heads" = ... = .... = what, of ours, joe enjoys messing with!

The clue breaks down like this:

=
knotted (an anagram of:)
under (under)
over-large heads (+ o & l)
So, anagram of underol = roundel
